Inpatient Rehab & Skilled Nursing


HOW DOES IT WORK?


Upon discharge from an acute hospital stay, some patients require further medical care to help them maximize their level of functioning, making them potentially eligible for the Skilled Nursing & Rehabilitation program. Hendry Regional Medical Centers, Inpatient Rehabilitation & Skilled Nursing program helps patients achieve this goal through its hospital-based provision of skilled nursing care and inpatient rehabilitation services. Our program provides rehabilitation and recovery services based on the patient’s individual needs.

WHAT IS THE EXPECTED LENGTH OF STAY?


Following discharge from an acute hospital stay, the length of stay depends on the patient’s medical condition and rehabilitation needs; typically, the time needed is about 14 days. However, each patient makes progress based on individual medical care needs and rehabilitation potential.

Our multi-disciplinary team will continuously monitor, record and formally review each patient's progress. As patients heal and can participate more in the program, the team will add, monitor and evaluate the patient with the goal of improving level of functioning to be able to be discharged home.

INPATIENT REHABILITATION & SKILLED NURSING PROGRAM REFERRAL PROCESS


Requirements for admission:

3-night inpatient stay (current or within the last 30 days at HRMC or another acute care facility)

Need for skilled service such as:: PT, OT, ST, daily IV antibiotics, daily wound care

Patient must be able to follow commands, agree to participate and not require a one-on-one sitter.
